Christa Pike's survival of Tennessee's failed lethal injection is drawing renewed scrutiny after a court filing cited testimony that execution teams did not practise inserting intravenous lines or rehearse their response to emergencies.
According to WSMV, attorneys representing Tennessee death-row inmates submitted a prison warden's 2025 deposition describing the training arrangements. The warden testified that the Department of Correction commissioner directed that IV lines not be inserted during practice sessions and that teams did not prepare for emergencies. The testimony adds to questions surrounding the state's September 30 attempt to execute Pike.
Pike survived after receiving two doses of pentobarbital. In a separate court account, her attorney described approximately 10 attempts to establish IV access, difficulty pushing saline through a line and warnings that veins might have ruptured before the lethal drug was administered. Those details represent the attorney's account; the investigation has yet to establish precisely why the execution failed.
Tennessee's Department of Correction previously maintained that personnel followed every step of the state's approved execution protocol. Governor Bill Lee subsequently ordered an independent review, and executions remain paused through the end of 2026.
For Pike, survival has brought no announced change to her death sentence. Asked whether Tennessee would attempt another execution or commute her sentence, Lee indicated that he expected the sentence to remain in place unless the courts determined otherwise. His remarks did not announce a new execution date.
Pike, sentenced to death for the 1995 murder of 19-year-old Colleen Slemmer, has returned to prison following hospital treatment. WSMV's latest account reports that she can speak in full sentences and walk independently.
The court filing now places execution-team preparation under sharper scrutiny as Tennessee reviews how its lethal-injection process failed-and considers what happens next.
